Location : Welland with travels

Salary range: $ 58, 412 to $ 70, 375 for Bachelor's degree

61, 821 $ to 74 482 for Master's degree

The incumbent will always work according to the principles of health promotion and the determinants of health. S/he will offer psychosocial evaluations and interventions as well as develop and implement brief intervention plans. In addition, S/he provide the appropriate referrals for clients with issues requiring long-term interventions.

RESPONSIBILITIES:
Collaborate in the implementation of an engagement and promotion plan for primary care providers in independent practice and their clients
Organize activities to raise awareness of mental health issues
Provide assessment and individual mental health intervention services to clients of physicians in independent practice in the Niagara Region
Contribute to client intervention plans by participating in case discussions and in collaborative care planning
Participate, as needed, in community initiatives and committees to improve the quality of life of clients with mental health issues
Participate in the development and application of intervention models that ensure quality services
Liaise with community agencies to improve access to appropriate services in a timely manner
Prepare reports, maintain records and maintain program statistics

Education, Experience and specific requirements:
Bachelor's degree or Master's degree in Social Work, Nursing, Occupational therapy or a related field from a recognized institution
Member in good standing of a Professional College (OTSTTSO (OCSWSSW), OPAO( CRPO), OIIO (CNO), OEO (COTO).....)
A minimum of three years of relevant experience
In-depth understanding of mental health issues
In-depth knowledge of psychosocial assessment and counseling techniques
Ability to handle crises effectively
Knowledgeable on the mental health community resources available in Niagara
Commitment to working co-operatively with clients and members of the interdisciplinary team
Openness to the customs and habits of other cultures
Good communication, organization, and interpersonal skills
Knowledge and ability to use basic computer software
Excellent knowledge of oral and written French and English
Be willing to work flexible hours to meet the needs of clients
Have access to a motor vehicle and hold a valid Ontario driver's license
