Your Opportunity:
As a Registered Nurse, you will provide front line interventions and a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. Integrated Home Care uses a strengths–based Client Centred Case Management practice framework. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. The successful incumbent will be expected to demonstrate proficiency in the following areas: Personal and professional accountability, Systems thinking, creative thinking, communicating and building relationships, Organizational and time management skills, Interpersonal skills, Intermediate level of computer skills.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:
Excellent organizational, oral and written communication skills; high level of independence and decision-making ability, as well as integrity for working with confidential information. Previous experience working with an interdisciplinary health care providers. This position requires a vehicle for business use. Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel is required.

Preferred Qualifications:
Two years of Home Care experience is preferred. Recent experience in a Community Health Setting as a RN Interventionist preferred. Previous community related experience a definite asset. A strong clinical background in Nursing will be considered.
