Disability Case Manager

The Disability Case Manager will perform and as a Health Screening Coordinator. The Disability Case Manager is responsible for assisting employees on behalf of the employer when the employee is off work or at risk of being off work for health reasons. The case manager assesses any barriers to a successful and timely return-to-work and engages with stakeholders to implement strategies to address those identified barriers and support return to work or an appropriate alternative plan. The Disability Case Manager will also be responsible for assessing and providing diagnostic screening tests to clients who participate in the wellness clinic. They will also provide back up support for the Nurse Clinician who is responsible for the emergent medical management of all Vale employees and applicable contractors, in an effort to provide prompt onsite management.

Roles & Responsibilities - Disability Case Manager

Conduct initial disability case assessment, meet with the employee and contact the work department to explain the disability management process and gather information for assessment purposes.
Develop and document an understanding of the situation and the factors supporting as well as inhibiting a successful return to work for the employee.
As required or appropriate, provide any of the following interventions: more in-depth assessment, medical management interventions, initiation and coordination of occupational and/or psychological interventions, utilization of specialized resources or consultations etc.
Provide active support for the return-to-work planning process between the employee and their supervisor as needed.
Conduct case management of interventions (occupational, non-occupational and plain) including, but not limited to - early intervention; education, counseling & awareness; employee advocate; collaborator; conflict resolution; liaison with internal and external resources and agencies; medical resource; adhere to policies and procedures as documented
Recommend early intervention of atypical results to ensure the utmost wellbeing of the employee.
Assist with scheduling and maintain management of all Critical Teams
Immunizations – workplace or work-related travel (If the candidate is a Registered Nurse)
Ensure compliance with applicable legislation such as Human Rights, WCB and OH&S legislation.
Coordinate and implement Smoking Cessation Program; meet with employees and/or their family members and develop an individualized quit plan. This includes education on effects of smoking, face-to-face and telephonic support, assistance with obtaining prescriptions for pharmaceutical aids, monitoring and maintaining statistics of success rate
Pre-employments; drug collection testing and breath alcohol testing
Work with the Manager to develop and improve medical file review procedures and accountabilities as well as other case management activity
Support nursing department with duties as necessary to ensure complete coverage of all service (disability management or medical surveillance) in the event of a colleague absence
Other duties as assigned

Qualifications - Disability Case Manager

Preferred education Registered Nurse or equivalent with active practicing registration with CRNM or
College or University degree and/or certificate or continuing education courses towards Disability Management, Occupational Health and Safety or similar industry designation

Current Certification in CPR
Relevant experience in Disability Case Management
Occupational Health Nursing Certification (an asset)
Strong interpersonal and helping skills
Superior assessment and problem solving
Proficiency in the use of Excel
Knowledge of disability benefits administration and adjudication
Valid driver’s license with access to a vehicle
Certified Industrial Audiometric Technician license (an asset)
Spirometry Technician certification (an asset)
Immunization Certification (an asset)

Working conditions

Works in an office/clinic environment, though occasionally will be required to travel to the VALE worksite for job assessments and/or meetings.
Must have the ability to tolerate Personal Protective Devices, including but not limited to – Hard Hat; Respirator; Safety Glass; Steel Toed boots.
Extended periods on the computer and telephone use
