The Opportunity

We are currently looking to hire a Assistant Care Manager for our Care Delivery department in Edmonton, AB. This opportunity will be a full-time permanent position. You’ll get to work with a growing organization where you will gain valuable experience in the field of home care!

The Assistant Care Manager is responsible for helping to grow the business in target markets, facilitate communication and act as a liaison with many of our stakeholders, provide medical expertise, manage service agreements for clients, oversee care plan development for clients, and manage a team of supervisors and front-line staff to ensure services are meeting client expectations. Specific accountabilities include:

Focus on growing billable hours in target markets with a focus on a long-term, sustainable strategy, specifically for medically and behaviourally complex in-home clients.
Facilitate communication and serve as a liaison between and among professional and direct care staff, family and/or guardians, and community medical providers and professional consultants in connection with CarePros.
Provide medical expertise to CarePros team and clients.
Directly manage the implementation, coordination, maintenance, evaluation, and termination of care plan services, including developing Positive Behaviour Support Plans (PBSP), while maximizing revenue for each Service Agreement.
Oversee the overall Care Plan development as well as 3-Tiered Care Team system for each client.
Oversees the responsibilities and duties of the Care Supervisors, and other staff as directed.
Facilitates communication directly with clients to ensure expectations are met and/or exceeded; works closely with client to update Care Plan (as required) to address changes in client’s medical condition or as new needs arise.
Coordinating with administration within various Program & Services under the Ministry of Human Services, including but not limited to Assured Income for the Severely Handicapped (AISH), Family Support for Children with Disabilities (FSCD), and Persons with Developmental Disabilities (PDD).
Drive growth initiatives through publicly funded sources (i.e. FSCD, PDD, AHS, Child & Family Services), insurance and disability sources (i.e. WCB, private insurance companies), and private pay.
Lead and collaborate with other Managers and Supervisors to onboard new clients in a timely manner while overseeing the proper creation and execution of individual service plans (ISP’s) or Care Plans for each client.
Participate and contribute to key strategic initiatives that will drive growth (i.e. expansion into other markets, exploring complementary services that will grow revenue, seeking and building strategic partnerships to drive mutually beneficial growth).

Who You Are

As a assistant manager in the home care field, you possess the following experience, knowledge, skills, and abilities:

5-7 years of supervisory or management experience in the home care or healthcare field
Knowledge of child and youth care mental wellness practices and the processes and procedures involved in providing effective home care.
Ability to actively listen, identify needs, and provide solutions to the client.
Recognize and understand emotions and handle situations with empathy.
Build and maintain cooperative and constructive relationships.

You have management experience in the healthcare industry along with a post-secondary education specializing in either Social Work or Nursing. A combination of education and experience will be considered.
