Pier Health is an award-winning Vancouver operation involved in multiple pilot projects on the cutting edge of our profession. We are a high energy, high volume, professional and awesome team that includes nurses, mental health workers, technicians and pharmacists. By providing daily medication delivery, witness, injection and general case management, our team helps keep at-risk citizens in their homes on a permanent basis. Our multi-disciplinary team works with Vancouver Coastal Health and Fraser Health teams across the city to deliver mental health, addiction and chronic disease programs.

We are looking for caring and compassionate Licensed Practical Nurses to provide care for mental health and addictions patients in Downtown East-side Vancouver. The successful candidates should be highly motivated to provide care and support for a vulnerable population that allows them to stay in their community. The care provided by the LPN will be focused on medication administration and witnessing. You will not be required to support ADL’s, transfer or personal care.

The successful candidates will have a valid BC Driver’s license. You will be provided with a company vehicle to perform your responsibilities; however, in the circumstances where a personal vehicle may be required (vehicle maintenance, repairs, etc.), you will be reimbursed mileage based on CRA regulations.

We are looking for LPNs with the following qualities:

  • Patient focused including going above and beyond to make positive contributions to the community in a healthcare setting
  • Willingness to learn and develop new skills

Responsibilities include:

  • Driving and visiting patients in their homes in the Downtown community with the purpose of administering medications (done orally, injections, creams and patches), collecting prescriptions, delivering paperwork and generally liaising between the patient and the Pharmacist
  • Carrying out assessments specific to each patient (examples include blood pressure and glucose monitoring)
  • Interpreting and documenting data from assessments in order to modify medications as required as well as all interventions
  • Follow organizational policies and procedures in critical situations
  • Documenting mental health patients behavioural changes
  • Reporting provided daily to applicable medical professionals
  • Communicating with medical professionals on substantial assessment information
  • Ensuring the most appropriate care is carried out for each patient to have the greatest positive outcomes
  • ADL’s, transferring and assistance with personal care is not required
  • Assisting affiliated mental health site Robin’s Pharmacy & Wellness centre for medication deliveries and support as required


  • BC Driver’s License
  • Licensed with BC College of Nursing Professionals (BCCNP)
  • Ability to provide injections under delegation from RN or physician
  • Ability to draw blood optional
  • Experience with the at-risk population an asset
  • Comfortable visiting at-risk residences
  • Criminal record check may be requested prior to hire


  • Full time – Tuesday through Saturday 7:00 am to 3:30 pm (40 hours)
  • Part time – Friday 8:30 am to 4:30 pm & Saturday 8:45 am to 1:45 pm at Royal Oak Methadone Clinic, Sunday & Monday 7:00 am to 3:30 pm Pier Health Resource Centre deliveries (28 hours)
