We are currently looking to hire a:

Nurse Practitioner – Community Health Team

4 Month Contract-Full Time (35 hrs/wk)

January 2020 to April 2020

Reporting To: Manager, Community Health Team


The Scarborough interprofessional primary care access model is a partnership to enhance care for the vulnerable, complex clients living in the Scarborough North and South Sub-regions. The priority population includes over 900 people currently on the Health Care Connect list, as well as non-insured and clients who require access to interprofessional teams.

Key areas of responsibility:

Working with our community-based primary care practitioners, our dynamic, collaborative and mobile inter-professional team will include Nurse Practitioners, Psychologist, RNs, RPNs, OTs, PTs, Social Workers, Dieticians, Navigator, Chiropodist, Foot Care Nurse, Case Worker, Mental Health and Harm Reduction Workers. This position introduces nurse practitioner position to our team.

To provide comprehensive primary health care for individuals and families living in the Scarborough community with an emphasis for service delivery on marginalized/at risk community members.

Educational and/or Professional Qualifications:

  • Master’s prepared Nurse Practitioner with Primary Health Care certificate
  • Registration, in good standing, with the College of Nurses of Ontario in the Extended Class
  • Member of NPAO with additional professional liability coverage
  • Minimum 5 years practice working as a NP utilizing advanced nursing assessment skills in an inter-professional model of care such as a CHC, FHT or NP lead clinic
  • Experience working as a Nurse Practitioner in a primary care setting is strongly preferred
  • Experience in program development preferred
  • Experience working with clients with mental health and additions considered an asset
  • Current HCP CPR
  • Demonstrated experience/confidence in working independently, with a high degree of self-direction and strong organizational skills
  • Demonstrated excellence in interpersonal, including effective conflict resolution, and communication (written and oral) skills
  • Proven ability to work as a member of the inter-professional team
  • Familiarity or willingness to practice with electronic health record documentation environment, Nightingale on Demand experience considered an asset
  • Sensitivity and awareness of culturally and racially, economically and socially diverse communities
  • Respect and adherence to agency policies, mission, vision and values philosophy of care
  • Strong research and planning skills
  • Capacity to maintain professional links with other agencies, colleges and universities.
  • Demonstrated commitment to principles of an anti-oppression and cultural competence framework;
  • Thorough knowledge of broad determinants of health and issues affecting low income, multi-lingual and radicalized and LGBTQ communities;
  • Oral/written fluency in other languages relevant to providing service in the role would be considered an asset

Remuneration: $ 47.40 - $63.37
