Position: Service Coordinator x 2
Department: one-Link, Mental Health Services
Status: Temporary Part-time for approximately 4 months (must provide availability of up to a .5 FTE)
Hours: Monday to Sunday – Days and Evenings, 8 and 12 hour shifts (Hours are subject to change based on operational needs)*

One-Link, led by Halton Healthcare, is a regional system access model funded by the Mississauga Halton Local Health Network Network as the centralized access to its ten funded addiction and mental health service providers. One-Link enables equitable access and coordinated care for individuals to receive the right combination of services, at the right time and place. Service Coordinators are tasked with providing access to treatment and supports and, as representatives of the addictions and mental health sector, ensuring every client and stakeholder receives an exceptional customer service experience.
This is what you’ll be doing:
Reporting to the Manager, Coordinated Access, the incumbent will work as a member of the professional team to provide the following:
Working closely with the Mississauga Halton Central Intake program, triaging of new referrals to determine appropriate next referral management steps
Standardized referral screening (primarily via telephone, occasionally via face-to-face or telemedicine) including level of care assessment through a utilization management tool (the LOCUS) for mental health and addictions
Matching clients to appropriate resources utilizing LOCUS results, service criteria, established standards, and clinical judgment
Follow-up with individuals and referrers as required to ensure timely and appropriate system navigation
Contribute to quality improvement initiatives to improve the services one-Link provides
Other duties as assigned

Bachelor’s degree from a social service/ healthcare program
Regulated Healthcare Professional (Registered Nurse, Social Work, Occupational Therapy) preferred
Member in good standing with your respective regulatory body
Minimum of 5 years’ recent experience within mental health and addictions required
Exceptional time management skills and attention to detail, including proven ability to follow complex processes, comfort with rapid change, and comfort with providing a balance of efficiency and client-centeredness within brief clinical encounters
Excellent interpersonal and communication skills and demonstrated ability to work independently and as part of multiple different teams both within the hospital and with community agencies
Advanced proficiency with Microsoft Office software applications; knowledge of Meditech and/or Novari e-referral software an asset
Proven exceptional customer service skills, including timely response to inquiries and appropriate follow-up, following a commitment to exemplary patient experiences always
Demonstrated ability to function with minimal supervision in a fast-paced environment
Bilingual (French) and/or demonstrated fluency in a second language other than English is an asset
Broad knowledge and previous clinical experience within concurrent disorders, addictions and withdrawal management, mental health, and supportive housing
Knowledge of the various service systems within the Mississauga Halton Local Health Integration Network
