Bridging Addictions Program

The Canadian Mental Health Association – Peel Dufferin Branch (CMHA/Peel Dufferin) is pleased to announce that we are currently recruiting for an experienced Concurrent Disorders professional for the position of Community Support Worker – Addictions.

As part of the Bridging Addictions Counselling program, this position will work in partnership with William Osler Health Centre (WOHC), Punjabi Community Health Services (PCHS), and Peel Addiction Assessment and Referral Centre (PAARC) to expand current Substance Abuse Treatment programs. This expansion increases the system’s ability to intervene through timely, high quality, integrated, person directed, culturally appropriate counselling and treatment services.

Initial and ongoing assessment, treatment planning, brief intervention, lifestyle and personal counselling will be provided to assist the individual to develop skills to manage substance abuse and related problems. Services will be provided in individual and group sessions.

This is a full time permanent position(37.5 hours per week). The incumbent will work in the catchment area of Brampton/Bramalea and report to the Program Manager - Bramalea care collaborative Team.

Salary Band: $50,700-$60,700

Key Responsibilities:
To work collaboratively with WOHS, PCHS, and PAARC to ensure responsive coordination for clients of Health Links and William Osler Health System’s Withdrawal Management Service.

In collaboration with the client, their significant others, and community partners create a plan for addiction treatment, considering the social determinants of health and the complex needs of clients affiliated with Health Links.

Facilitate referrals to residential, day, mutual aid, self-help, and or out-patient/community addiction services, as well as other supportive services as required.

To follow up with clients to ensure that seamless linkages are in place to optimize treatment outcomes.

To conduct risk and psychosocial assessments using validated tools.

To engage clients, using motivational counselling and other modalities, within a harm reduction and stages of change framework.

To provide individual and group counselling.

To establish crisis management plans with each client and to provide crisis intervention as necessary.

To assist with the development, implementation, and evaluation of the program.

To utilize required data collection and reporting systems.

To complete all client documentation, as required

Will participate in staff and other meetings, both internal and external, as required.

Skills & Qualifications:
University Degree in Social Work or related discipline.

BSW is preferred.

Post graduate addictions counselling diploma is required.

Registration with affiliated College is required.

Minimum of three years addiction (concurrent) counselling experience is required.

Demonstrated cultural competence and the ability to support a diverse client population.

Demonstrated skill in addiction assessment and treatment.

Demonstrated experience in developing and facilitating groups.

Ability to work independently as well as with multi-disciplinary teams, both internally and externally

Excellent oral and written communication, organizational and problem solving skills

Knowledge of Client Record databases and addictions assessment tools.

Knowledge of French or any other second language is an asset.

Valid “G” Driver’s license, relevant insurance and use of reliable vehicle.

A satisfactory Vulnerable Sector Screening (Police Check) report.

Current CPR/First Aid certification.
