Personal Support Care Team Supervisor - Toronto Central

Permanent Full-time

The PS Care Team Supervisor leads and supports a team of PSWs ensuring the delivery of client and family-centered personal support services that meet VHA’s standards of excellence. You will conduct initial home visits with clients to establish appropriate care plans . You will provide ongoing clinical support, supervision, education and training at the bedside to a large Personal Support Care Team who are supporting clients to achieve more independence.

Key Skills, Experience and Behaviours required for this position:

A RN or RPN in good standing with the College of Nurses with exceptional clinical assessment and communication skills who can interact in a professional, culturally competent manner with clients, funders, staff teams and community partners.
3 plus years of experience required. Community and home health care experience preferred.
A leader and strong team player who promotes Client and family-centered care to support client and employee engagement.
Highly organized and comfortable working in a constantly shifting environment as VHA responds to changes in client needs.
Able to work effectively under pressure and adheres to deadlines.
Able to write clear and concise field and supervisory reports.
Advanced experience with Microsoft Office and a keen desire to bring technology to the bedside.
Ability to work independently and as a member of a collaborative team.
Ideally, prior supervisory experience within a unionized setting and know what it takes to uncover talent and innovation as you build and inspire a highly engaged large team.
As a supervisor, you have working knowledge of relevant employment statutes including the OHSA, ESA and OHRC.
A valid Driver’s License and access to a vehicle are required.
Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, pushing and pulling.
A proven track record of effectively handling complaints and resolving conflicts.
Demonstrated efforts to continually develop and upgrade your skills.
Ability to converse in a second language is a definite asset.
Flexibility in scheduling is required, including days, weekends, evenings, and holidays.
Supervisory On-Call on a rotational basis and as required.
