Reporting to the Program Coordinator, this position works with the health care team within the context of a person and family centred recovery model, to carry out individual or group social activities and established nursing/personal care programs. Duties include carrying out individualized and group social activities, assisting the client with practicing daily living skills, accompanying clients on group outings, observing and reporting client progress, participating in client focused discussions and attending continuing education in- services. Provides personal care to clients such as bathing, grooming, dressing, toileting and feeding.

Grade 12, graduation from a recognized Resident Care Attendant Program plus 2 years’ recent experience working with older adults with behavioural and psychological symptoms of dementia (BPSD) or an equivalent combination of education, training and experience.
Successful candidates will be required to successfully complete a Food Safe certification course during their orientation/qualifying period in order to be deemed to have successfully passed their qualifying period.
As determined by the Program Coordinator, carries out individual or group leisure activities such as arts and crafts, woodworking, gardening, sports, games, socio-cultural events and social outings, both inside and outside the facility, by methods such as selecting appropriate activities, confirming details of proposed activity with Program Coordinator communicating the activity to clients, their families and staff, accompanying clients on outings, coordinating the supervision of clients during the activity and provides clients with opportunities for physical and functional mobility according to established procedures.
As determined by the Program Coordinator assists clients with practicing daily living skills such as personal care, ambulation, and independent use of wheelchairs by performing duties such as providing physical support, assisting client with transferring and supervising or demonstrating techniques designed for a specific client to assist the client in reaching their goals.
Under the functional direction of a Registered Nurse and in accordance with established procedures, delivers care for assigned clients by referring to wellness plans, provides personal care to clients such as bathing, grooming, dressing, oral hygiene and assisting with toileting needs, identifying client/family priorities/plans for discussion with the Registered Nurse, reorganizing priorities, providing information to revise wellness care plans as required and including client/family in evaluating progress towards their goals..
Observes, monitors and reports client progress, symptoms and behavioural changes through methods such as recognizing and responding to anticipated/unanticipated client responses to care and reporting significant observations such as client discomfort and/or pain, skin breakdowns, and unusual incidents and/or accidents to designated personnel; provides suggestions for change or modifications to existing programs.
Participates in program discussions and in-services through methods such as attending interdisciplinary meetings to provide/receive information on the planning of care and development of the wellness plan for clients, drafting/revising documentation methods/forms for review by committee/team and providing written/verbal input to promote improvements to client and family care, and the efficient and safe operation of the program.
Assists clients with meals by performing duties such as serving meal , checking meals to ensure the maintenance of dietary restrictions, and feeding designated clients.
Establishes therapeutic relationships with clients and families and provides emotional support to clients through methods such as identifying client’s and family’s perspectives, learning needs and readiness and ability to learn. Initiates discussion to identify what is important to the clients/families, and clarifying what life is like and understanding their concerns, wishes and goals.
Performs other related duties as required.
