Nurse Next Door Home Care in Nanaimo is looking for another great team player to assist in managing our Client Care as a Care Coordinator. The Care Coordinator will support in leading and delivering Premium Care to our clients and a premium experience to our caregivers in a largely administrative role. LPN certificate credentials at a minimum are required.
In this role you will enjoy working collaboratively, but with a high degree of autonomy and
responsibility. Seeking optimal solutions for each care situation and able to manage both simple
and complex care situations from Initial Caring Consult to completion of care. Approximately
50-75% of the role will be in an office setting, utilizing your strong admin skills via computer,
smartphone, online programs, and more technology. A moderately strong admin and
technology background is necessary. The ability to stay calm in situations where many tasks are
requiring attention and the ability to prioritize and focus on the task at hand is essential and

Must be able to work Saturdays
Must be proficient with technology, social media, typing, and smartphone usage
Must be driven and self-motivated
Client & Caregiver Experience:
Ensures that the First Visit introduction to the client and caregiver is the best
representation of the Nurse Next Door Brand, culture and premium care experience.
Collects & verifies Service Agreements from Clients if applicable
Reviews and confirms the Client’s schedule to clarify any questions
Reviews Care Binder with the caregiver, this includes
Identifies training needs for caregivers
Premium Experience - Continuity of Client Care:
Provides training and support for caregivers as needed
Follows up with CarePlan and update as needed
Identify changes in the level of care and communicates recommendations with Care
Designer (Manager) in a timely manner i.e. has the level of care changed?
Premium Experience - End of Care:
Assesses why care is ending and connects with client/caregiver to communicate change
in a timely manner
Completes final assessment and follow up with any feedback
Collects and reviews CarePlan and includes final notes
Discharges client files in a timely manner
Assist our scheduling team with coordinating caregivers/client schedules
Monitors emails to respond to scheduling inquiries/changes/cancellations and provides
recommendations to the scheduling team when needed.
Filling In for occasional & urgent cancellation visits when client coverage is required / essential
EDUCATION and/or EXPERIENCE REQUIREMENTS:
LPN (Licensed Practical Nurse) Certification up to date
Valid BC Drivers License (Class 5) with clean driving abstract
Up to date CPR / First Aid training
Exceptional written and verbal communication skills especially communicating with
families, physicians, and medical personnel.
Superb organization, planning, time management, and multi-tasking skills
Clear Criminal Record Check w/Vulnerable Sectors Screen included
