The Hematology and Oncology Program McMaster Children’s Hospital provides tertiary subspecialty oncology and hematology care to children ages 0-18 years. This position includes coverage in both Inpatient and Outpatient care settings.
The Pediatric Hematology/Oncology Clinical Pharmacist provides comprehensive pharmaceutical support for inpatient and outpatient pediatric Hematology/Oncology patients. Areas of focus include monitoring drug therapy to evaluate appropriateness of use, monitoring and reporting adverse drug reactions and medication errors, writing inpatient and outpatient chemotherapy orders, writing pre and post chemotherapy hydration and adjunct medication orders and providing support to the interdisciplinary team regarding medication administration and related pharmaceutical questions. The pharmacist also provides education to parents and patients about chemotherapy and supportive care.
Schedule Work Hours
Monday to Friday; days (clinical – 80%)
Some evenings, weekends, holidays, and on call (central pharmacy coverage – 20%)
1. Licensed by the Ontario College of Pharmacists to practice Pharmacy in Ontario
2. Baccalaureate of Science Degree in Pharmacy
3. Minimum of 2 years experience in a hospital setting demonstrated ability to work in an interdisciplinary team environment
4. Advanced clinical training in pharmacy, such as a pharmacy residency program or Pharm. D. preferred
5. Demonstrated experience in professional and distributive activities unique to pediatric and oncology settings within a teaching hospital preferred
6. Knowledge of pediatric and oncology based pharmacotherapy
7. Evidence of continuing education activities
8. Demonstrated communication skills, teaching and evaluation skills
9. Computer skills including pharmacy systems, word processing, spreadsheet and email systems
10. Self-directed practitioner willing to share clinical skills
