This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.
Reporting to the Health Services Manager, Cancer Care & Palliative Care Services or designate, the Palliative Care Consultation (Specialist) Nurse (PCCN) provides a specialized consultative nursing service to individuals who are living with a life-limiting illness. The PCCN is a member of the intra-disciplinary Palliative Care Consultation (Specialist) Team. The PCCN completes comprehensive assessments and works in partnership with interprofessional care teams, primary care providers, community agencies and volunteers in addressing the full range of physical, psychological, social and spiritual needs of the individual. This includes a strong emphasis on the provision of skilled and timely pain and symptom assessment and management. The PCCN recognizes the patient and family as “the unit of care”.
The PCCN provides clinical leadership that is focused on improving the quality of care delivered to individuals and family members coping with life-limiting illness. The PCCN serves as an educator, coach and mentor in the provision of the palliative approach to care.
The PCCN exhibits sound professional judgment and initiative while working within the Mission, Vision, Strategic Directions and incorporates into daily practice the Core Competencies and Values of the organization.
Graduate of an accredited School of Nursing Program
Registered or eligible for registration with the Nova Scotia College of Nursing (NSCN)
Minimum of five (5) years continuous nursing experience required
Work experience in the care of palliative patients and families essential
Palliative Care Consultation (Specialist) Nurse experience an asset
Certification in Hospice Palliative Care Nursing (CHPCN) required or willing to become certified within two years of hire
Mental Health First Aid Training an asset
Demonstrated ability to understand and utilize the domains of care associated with illness and bereavement
Excellent physical assessment skills including pain and symptom assessment/management
Experience organizing, leading and facilitating in services, education programs and group meetings
Demonstrated ability to work autonomously and as an effective team member
Demonstrated leadership skills including effective decision-making, critical thinking and problem-solving skills required
Excellent oral and written communication skills, with the ability to demonstrate interpersonal effectiveness with patients, families, volunteers and other healthcare providers including family physicians/nurse practitioners,.
Ability to facilitate patient/family/team conferences in various settings
Exhibits characteristics of warmth, empathy, humor and a non-judgmental philosophy of care
Physical and mental capabilities to perform the duties of the position such as the ability to tolerate high stress levels, including multiple losses; adaptable to flexible and variable work hours
Commitment to promoting a culture of respect that supports safety, ethical practices, cultural awareness and organizational health
Demonstrated competency in basic computer skills (Microsoft Suite, E-mail, Internet, Excel, SHARE, and Meditech); and the ability to acquire additional skills as necessary (e.g. SEASCAPE)
Minimal travel within the geographical area may be required
Valid Nova Scotia Driver’s License and/or daily access to a reliable vehicle
Competencies in other languages an asset, French preferred.

Hours of Work
Long Assignment Part-time (40%) Position; 30 hours bi-weekly
Beginning January 6, 2020 with an expected end date of January 7, 2022
Dates are approximate and subject to change
Shifts include days, nights, weekends & holidays
$35.0356 - $41.2784 Hourly
($42.7230 with 25 years' RN experience)
