We are recruiting for the following geographies in Nova Scotia:

Bedford, Hammond Plaines, Prospect, Spryfield, Halifax Penn, Clayton Park.

As a member of Closing The Gap Healthcare Group, you will provide Continuing Care Services to Adults in a community setting. This is an opportunity to work 1:1 with clients and work independently.

Experience & Credentials for:


  • Must not have a criminal record (acceptable criminal check and child abuse registry check)
  • Provincial certification as a Continuing Care Assistant (and maintained)
  • Must have First Aid Certification and CPR Certification

CCA in training

  • Must not have a criminal record (acceptable criminal check and child abuse registry check)
  • Successfully completed CCA program modules: Introduction to the Continuing Care Assistant Program, Body Mechanics: Positioning/Transfers, Personal Care
  • Must complete certification as a Continuing Care Assistant with the next two years
  • Must have First Aid Certification and CPR Certification

Additional roles and requirements:

  • The provision of personal care to clients in their homes
  • Travel within urban and rural settings (must arrange own transportation)
  • Ability to document and have basic computer skills
  • Provide exceptional customer service through strong interpersonal skills
