Ontario Aboriginal Housing Services (OAHS) provides safe and affordable housing for First Nation, Métis, and Inuit people living in urban and rural areas of Ontario. Housing Services are provided throughout the province by over 75 Team Members as well as a wide range of Partners including Indigenous Organizations, Community Organizations, Governments (Municipal, Provincial, Federal), and Private Sector. Our Main Office and Service Centre is located in Sault Ste. Marie and we have Satellite Service Centres in Dryden, Peterborough, and Hamilton. Services provided include:

OAHS continues to expand by adding a range of housing services. As a result, we are seeking an "Aboriginal Housing Based Case Manager” for our Operations team located in Sault Ste. Marie, ON.
Reporting to the Resilience Coach/Supervisor the Aboriginal Based Case Manager, will be responsible for, but not limited to, supporting homeless people to become housed and learn relevant skills, facilitating access to culturally appropriate supports, and in supporting long-term housing retention.
Duties and responsibilities will include, but not be limited to the following:
· Meet with clients in shelter, refer and support clients to access culturally appropriate community supports/services according to their needs (addiction, mental health, legal etc.)
· Accompany clients or arrange accompaniment to appointments that are essential to their progress as required
· Provide case management services to aboriginal clients housed under the urgent homeless category as determined by Housing Programs staff
· Initiate a support service plan that will address safety, basic needs, relationships, daily activities, life skills, social and community integration, risks and triggers, managing tenancy, economic and
wellness opportunities, and create goals for greater independence including exit planning from case management services
· Assist clients in accessing clinical supports and support treatment plans, liaise with treatment professionals
· Complete the Service Prioritization Decision Assistance Tool with clients at the prescribed intervals to identify goal planning areas, on-going housing issues, and the degree of supports required - reporting results to Housing Programs staff
· Support the landlord/tenant relationship, following up with tenant/landlord to ensure the rent is being paid.
· Conduct regular home visits in order to support clients to maintain their housing unit in good repair by monitoring levels of cleanliness, teaching or reinforcing life skills, and referring to community supports.
· Assist in resolving issues that put the tenancy at risk, including advocating with other services, and having a solution focused approach
· Help clients prepare the following: monthly budget, weekly calendar of meaningful things to do, Personal Guest Policy, Risk Minimization Plan, and Crisis Plan
· Connect clients to resources in the community that assist in managing their tenancies, or provide long-term support, as required.
The successful candidate will have the following qualifications and skills:
· University degree in Social Sciences from a recognized university (preferred)- College diploma in Social Services or other discipline may be considered.
· Three-years of experience in a position involving direct client services/support, one-on-one
and /or group counselling
· Thorough knowledge of the social support services/resources network in our community
· Familiar with the Housing First Model
· Valid driver’s license with access to vehicle
· Excellent oral and written communication skills
· Strong problem-solving skills and the ability to use sound judgment
· Ability to work with minimum supervision and as a team member with a keen desire and focus on continuous performance improvement
· Excellent knowledge of program development as related to Aboriginal Housing programs; and understanding of current Aboriginal Housing programs and agreements;
· Excellent ability to communicate professionally (oral and written) using multiple channels with a variety of stakeholders both internal and external to the organization;
· Strong understanding and appreciation as to how safe, affordable housing contributes to quality of life and serves as a stable foundation; and an
· Appreciation for Aboriginal culture and a values system that believes in assisting people.

· Ability to travel as required (limited)
· Valid CPIC clearance (Criminal Records check);
· Must be Bondable;
· Current, valid Ontario Driver’s Licence and safe driving record.
