Stonegate Community Health Centre is a not-for-profit; community-governed organization that provides primary health care, chronic disease management, health promotion programs and a variety of services and activities for people of all ages. Stonegate is committed to serving individuals and families who experience barriers to accessing health care services.
This is an 18 month maternity leave contract with an annual salary of $42,736.00, paid vacation and benefits.
The goal of the Early Years Program Facilitator position is to support the healthy development of children and families through delivering high quality programs that address the determinants of health.
Acting as a member of the interdisciplinary team the Early Years Facilitator works collaboratively with team members in the development, implementation, operation and evaluation of Early Years services and activities for families in the communities served by Stonegate Community Health Centre.
The Early Years Facilitator is a member of the Family Program and broader Community Program Team which is comprised of a multidisciplinary team that includes the Health Promotion Team. The team works collaboratively to deliver programs within a health promotion framework that provide supports and resources to prompt or enable behaviour change.
This role also has a strong emphasis on reducing isolation, promoting healthy family relationships and the optimal development of children. This position works closely with other partner agencies to maintain a positive environment and collaborative partnership.
The Early Years Facilitator reports to the Manager Health Promotion.
Collaborate with members of the Family Programs team to ensure program activity design and implementation is in agreement with the program logic model and health centre objectives.
Increase community awareness of the importance of the early years and healthy child development.
Coordinate program activities and community events for children of all ages.
Design and deliver parenting, early years and health promotion programs to promote the development of social support networks and increase participant knowledge and skills.
Work in partnership with families to assess their needs and respond to them in a supportive and professional manner
Ensure that program evaluations are completed and summarized.
Actively engage in community outreach to local schools and agencies to promote Family Program activities.
Develop links with internal and external programs to make appropriate referrals and to provide information about community programs and services.
Develop displays and resources .
Supervise and evaluate student/volunteers
To be welcoming and non-judgemental with individuals and families participating in program activities.
Support the development and implementation of child and youth program as needed
Maintain accurate program statistics and records ensuring group data entry is completed within 5 days of the group occurring.
Develop and implement an annual work plan and work in partnership with members of the Family Programs team to develop an annual work plan for the team.
Ensure the timely preparation and completion of program evaluations, annual program summaries, proposals, presentations, and funding reports as requested.
Assist in the research and preparation of grant applications.
Acts as a staff resource, ensuring and providing coverage for other members of the Family Program team as needed.
To consult with the Family Programs Manager regarding program expenditures and purchases.
Actively participate in collaborative program and health centre projects, work groups and committees as required.
Work collaboratively with the Data Management Coordinator to ensure that Family Programs data entry, evaluation and data reports are produced in a timely and meaningful fashion.
Community Outreach and Health Promotion:
Establish, maintain and support external partnerships as applicable to the work of the Family Programs team.
Identify appropriate community outreach and educational strategies.
Assist in the identification and assessment of health centre needs.
Make recommendations and participate in the implementation of health education/promotion programs for the community.
Promote the development of social support networks among parents, caregivers, and children.
Assist in the orientation of new Family Program team members and casual staff.
Establish and maintain positive partner relationships relevant to the Family Programs team.
Provide support, mentor and evaluate student and volunteer placements.
Work in partnership with members of the health centre team to deliver programs and services to the local community.
Maintain and develop professional competence through appropriate professional development and continuing education.
Ensure all program delivery, materials and presentations reflect an understanding and knowledge of the social determinants of health and the centre mission and strategic directions.
Minimum 1 to 2 years of experience as a RECE
Knowledge and experience related to the design, implementation and evaluation of programs for children 0 to 6 years of age.
Understanding and knowledge of family support principles and how they relate to a health promotion framework.
Experience developing and facilitating programs for families.
Early Childhood Education Diploma or Degree
Registered and in good standing with the College of Early Childhood Educators
Creative, resourceful, excellent interpersonal skills, able to work collaboratively with others, good group facilitation skills, excellent understanding of child development, family support principles and issues facing families today.
