The role of Program Manager – Continuing Care Transitional Spaces (PM), is an amazing opportunity for a candidate who enjoys working in a fast paced environment where maximizing access to support client care is paramount. You will support daily operations and decision making for bedded respite, post-acute options, Day Programs and the CHOICE program. The PM will ensure there is strong collaboration with all stakeholders in addition to providing leadership and strategic direction for the Transitional Spaces operations coordination team. The PM will be accountable for overseeing - 1840 day program spaces/week, 29 respite and CHOICE holding beds and 310 post-acute spaces; these spaces are located at 44 different sites. The PM is accountable for standardization across different locations by providing direction and oversight through an informal reporting model with non-AHS based Unit Managers and Program Leads. Key Responsibilities: reviewing the CHOICE Program and the development of a new CHOICE site in the Edmonton Zone; overseeing respite options including ADP, CHOICE and bedded options; overseeing post-acute program options including sub-acute, restorative and transition unit spaces; reviewing transitional bed utilization, including creating strategies to improve access, availability, and occupancy and to determine current/future needs of facility based beds for both urgent and planned use; providing coverage support and back-up for the Director, Transition Services.
Continuing Care Services (CCS) has invested substantial effort into creating workflows to support Destination Home, enhanced home care, and caregiver respite options. Bachelor’s degree in a health related discipline–Master’s level preferred. Minimum of 5 years experience working in continuing care. Minimum 5 years of progressive leadership/management experience. Experience working with multiple organizations. Annual membership in good standing with an accredited professional association.
