Our Home Care team strives to provide client & family-centred care to our clients in their homes in community/lodge/supportive living settings. We value client and family involvement in care planning and decision-making. Our Case Managers provide care planning, oversight and some care to our clients in community/lodge/supportive living settings. The Case Managers also direct the services delivered by our Health Care Aides (HCAs) and LPNs who provide care to our clients who live in community & lodge settings. Our HCAs provide authorized services which can include but not limited to personal care & medication assistance to our clients. All Home Care staff is required to have a vehicle, with business insurance, for work purposes because our staff travels throughout the greater Grande Prairie area to provide service. This position is currently (subject to change as required) in community and serving Hospice. If you enjoy working autonomously within a supportive office environment, this job may be for you. You will be working in conjunction with LPNs as well as oversee authorizations for the HCAs. This type of nursing requires someone with a strong skillset in assessment and decision-making. You will be working with clients from birth to end of life spectrum. You will be required to work collaboratively within a multi-disciplinary team as well as with the clients and their support networks. You must have well-developed critical thinking skills. Experience dealing with all entire age spectrum including peds and seniors. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
