Pathways to Independence is hiring One (1) Support Worker, Renfrew who will work as a member of the support team to enhance the social, leisure and daily living activities of clients. Under the direction of the Client Services Manager, the Support Worker supports adults’ mental health, behavioural challenges and those involved in forensic services in accordance with the person’s individual Quality of Life Plan. The Support Worker assists with activities such as homemaking, personal care, community and personal development skills in accordance with the person’s Quality of Life Plan and other directives, as required. The Support Worker will implement established strategies and behaviour management plans under the general directions of the Manager and Counsellor. The Support Worker ensures that the person’s services and supports promote their rights, choices, self-determination, independence and builds personal and community living skills.
The successful candidate must have the following skills/qualifications:
- Secondary school graduation and a minimum one year post-secondary certificate in Human or Health Services (Personal Support Worker or equivalent).
- Current First Aid and CPR;
- Valid G class driver’s licence.
Ideal candidates will also have experience and abilities aligned with the key responsibilities for these positions, which includes:
- Demonstrate a person centred philosophy and commitment to agency vision, mission and guiding principles;
- Experience working with adults with a developmental disability in a community based, group living or independent living environment;
- Provide effective services and supports to supported persons;
- Manage and maintain accurate client information and documentation;
- Participate in and ensure a positive team environment.
