Position Overview
Strengthening the primary care infrastructure within the Winnipeg Health Region is a key priority of the Winnipeg Regional Health Authority and the province of Manitoba. The vision that Manitobans will have knowledge of and access to high-quality, cost-effective primary care is a long-term vision and several strategic actions have been identified to move towards this goal. These actions are based on working in partnership with private practice fee for service family physicians. Several strategic actions have been identified to accomplish this priority, and involve partnerships with community health services, primary care providers, the provincial government, private practice family physicians, and others. Key strategies include primary care network development and facilitation, the regional Family Doctor Finder (FDF) program, and the enhancement of interprofessional practice in primary care settings, linkages to specialty services to support enhanced access to quality comprehensive care for patients in community. Within this context, the WRHA is currently working with partners to facilitate the introduction of interprofessional teams in fee for service practices, develop after hours primary care services, connect patients with no identified primary care provider to primary care homes, and service navigation and linkages to specialty services. Other areas of strong connect are with the Chronic Disease Collaborative, Mental Health Program, and Home Care Teams.

Under the general supervision of the Director Primary Health Care, WRHA Primary Health Care Program and as a member of the FDF Team the incumbent implements and manages the regional FDF Program,. Works closely with members of the Primary Health Care Program team, relevant sites, community organizations and other stakeholders to develop and implement strategies that ensure the alignment of Care connection with primary care renewal activities including: inter-professional in fee for service practice, My Health Teams (MyHT), Physician Assistants in Primary Care, Electronic Medical Record implementation and optimization, Primary Care Home Clinic and enrolment.

A minimum of five years related experience working in the area of primary health care, with experience and knowledge of physician engagement principles required.
Five years recent clinical experience required.
Demonstrated strategic leadership, program coordination and management experience required. A sound understanding of population health, primary care clinical and inter-professional practice is required.
Experience working in private family medicine practice and working with private practice physicians required.
Education (Degree/Diploma/Certificate)
Bachelor's degree in a health-related discipline required, Masters’ degree in a related area preferred
Other suitable combinations of relevant education and experience may be considered.
Responsible for maintaining and providing proof of active registration with the appropriate registering body.
Valid class 5 Manitoba driver’s license and access to a reliable vehicle, with ability to travel within Manitoba.
Qualifications and Skills
Competent in Windows-based computer programs (Word, Excel, PowerPoint, Outlook).
Bilingual preferred.
Program planning, development, implementation and evaluation with particular understanding of primary care renewal and strategic actions within the national, provincial and local context.
Experience working with diverse populations and/or community health services, across various health disciplines required.
Thorough knowledge of the healthcare system at the regional health authority level and within the Province of Manitoba required, including knowledge of: processes and structure of regional healthcare programs and services; cultural and other demographic influences on regional program and service needs; structures, daily processes, challenges and issues in a primary care clinic-based environment; available internal, external and community resources, with the ability to effectively utilize resources to meet organizational objectives.
Knowledge and experience in the area of human resource management, budget and financial and statistical reporting systems.
Demonstrated leadership in and interdisciplinary team environment.
Excellent organizational, interpersonal, problem-solving and decision-making skills.
Ability to evaluate evidence and literature.
Strong facilitation skills and knowledge of the principles of adult learning.
Ability to work independently within an interdisciplinary and intersectoral team environment.

Salary is commensurate with education and qualifications.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment.
