The CSC is a multidisciplinary non-profit center providing health, community and social services. We have
two main points of service, one in Hamilton and the other in Welland. We are looking for professionals to fill

the following positions:
SOCIAL WORKER OR PSYCHOTHERAPIST - CARING FOR MY COPD

PERMANENT AND PART-TIME POSITION - 7 HOURS / WEEK
WORKPLACE: Welland

$14,076 TO $14,895
Under the direction of the Specialized Services Director and the supervision of the Caring for my COPD
Coordinator, the incumbent will always work according to the principles of health promotion and the
determinants of health. He / she will offer psychosocial evaluations and interventions to a primarily geriatric
population as well as develop and implement brief intervention plans. In addition, he / she will provide the
appropriate referrals for clients with issues requiring long-term interventions and will participate in
education.

RESPONSIBILITIES :
  • Provide assessment and individual mental health intervention services to clients of the Caring for my
COPD program

  • Works collaboratively with other Caring for my COPD team members
  • Develop and lead educational and therapeutic groups to improve the quality of life for clients with
obstructive lung disease

  • Participate in the development and application of intervention models that ensure quality services
  • Liaise with community agencies to improve access to appropriate services in a timely manner
  • Prepare reports, maintain records and maintain program statistics
  • Provide individual interventions by using diverse therapeutic approaches such as: behavioral and
cognitive behavioral therapy, ensuring approaches are evidence based

SPECIFIC REQUIREMENTS:
  • Bachelor's degree or Master's degree in Social Work or a related field from a recognized institution
  • Member in good standing of a Professional College
  • Developed sense of organization and in-depth understanding of mental health issues
  • Ability to handle crises effectively
  • Knowledgeable on the mental health community resources available in Niagara
  • Openness to the customs and needs of diverse populations
  • Good communication and interpersonal skills
  • Able to use basic computer software
  • English skills (oral and written)
  • Knowledge of French will considered as an asset
  • Experience working with geriatric population an asset
  • Experience providing CBT education an asset
  • Have a valid driver's license and have access to a motor vehicle
  • Minimum 3 years of experience preferred
