Vita Community Living Services & Mens Sana is a not for profit charitable organization funded primarily by the Province of Ontario to provide services to individuals with developmental disabilities. In 2005, Mens Sana Families for Mental Health joined Vita. Today, both organizations, though remaining separate corporations, are managed and governed by a common organizational structure and policies. Each organization has a separate Board of Directors, although the membership is the same on both boards. The two organizations are referred to as “VITA”. The organization provides residential, crisis, respite and day services to adults with developmental disabilities and mental health issues. VITA employs over 400 full-time, part-time and relief staff and has an operating budget exceeding $20 million.

Job Responsibilities (list is not exhaustive of responsibilities):
1.Develops and demonstrates an empathetic working relationship with the member for whom he or she is hired to work with.
2.Develops, implements and evaluated Personal Support Plans with the involvement of the members to enhance their overall life skills and quality of life.
3.Identifies members’ need for specialized services; through consultations and referrals, arranges appointments, transportation and supportive accompaniment;
4.Arranges appointments, transportation and supportive accompaniment and completes follow up documentation as necessary for specialized services
5.Supports members in identified areas of work skills, recreational skills, social skill and life skills, and when necessary assists members with personal hygiene matters
6.Administers and records prescribed medication as per instructions from medical practitioner and follows the agency’s Medication Policies and Procedures.
7.Communicates any medical needs of the member to the appropriate personnel, applies first aid as required and completes written follow-up documentation.
8.Provides feedback regarding member’s general health and safety issues to the Program Supervisor and completes documentation as necessary.
9.Orders, administers, and records prescribed medications as per instruction from medical practitioner.
10.Ensures the health, safety and welfare and independence of the residents by completing and involving the member with general house cleaning, grocery and supply shopping, the preparation of meals andlaundry as necessary.
11.Assists in organizing leisure activities and ensuring community integration for all members, in accordance to their needs and interests.
12.Transports and/or facilitates transportation to day program, family visits, community outings and medical appointments.
13.Performs other related duties such as balancing petty cash, vehicle maintenance, completion of fire drills, facilitation of observations shifts and assisting with orientation of new members, staff, students, volunteers.
