As the Registered Nurse - Community Care Case Coordinator, you will act as a close liaison with family members, significant others, physicians and health care providers in acute, long-term and community care, while assessing, planning, implementing and evaluating care for clients using nursing diagnosis. As a Case Coordinator, you will report to the Manager of Community Care and will use a case coordination approach while assisting clients and their families with providing assessments, case coordination and direct care for a caseload of home care clients. You will demonstrate a professional, positive attitude while continually utilizing your excellent communication, interpersonal and problem solving skills. Experience in Meditech, Care Manager and RAI. Recent nursing experience in Senior's Health.
