Vita Community Living Services & Mens Sana Families for Mental Health are two not for profit charitable organizations (which amalgamated in 2005) which provide services to individuals with developmental disabilities and/or mental health challenges . The two organizations are referred to as “VITA”. The organizations provide residential, crisis, respite and day services to adults with developmental disabilities and mental health issues.

Job Responsibilities (list is not exhaustive of responsibilities):

1.Develops and demonstrates an empathetic working relationship with the member for whom he or she is hired to work with.

2.Develops, implements and evaluated Personal Support Plans with the involvement of the members to enhance their overall life skills and quality of life.

3.Identifies members’ need for specialized services; through consultations and referrals, arranges appointments, transportation and supportive accompaniment;

4.Arranges appointments, transportation and supportive accompaniment and completes follow up documentation as necessary for specialized services

5.Supports members in identified areas of work skills, recreational skills, social skill and life skills, and when necessary assists members with personal hygiene matters

6.Administers and records prescribed medication as per instructions from medical practitioner.

7.Communicates any medical needs of the member to the appropriate personnel, applies first aid as required.

8.Provides feedback regarding member’s general health and safety issues to the Program Supervisor

10.Ensures the health, safety and welfare and independence of the home by completing and involving the member with general house cleaning, grocery and supply shopping, the preparation of meals and laundry as necessary.

11.Assists in organizing leisure activities and ensuring community integration for all members, in accordance to their needs and interests.

12.Transports and/or facilitates transportation to day program, family visits, community outings and medical appointments.

13.Performs other related duties such as balancing petty cash, vehicle maintenance, completion of fire drills, facilitation of observations shifts and assisting with orientation of new members, staff, students, volunteers.


  • Full-time Direct Care Worker 2 - D.S.W, S.S.W and C.Y.W, and/or related post-secondary education
  • Part-time Direct Care Worker 1 - Actively working towards a D.S.W, S.S.W, C.Y.W.
  • Direct Care Worker Assistant- P.S.W. Diploma
  • Overnight awake/sleep- P.S.W. Diploma
  • ADDITIONALLY (for all positions)
  • Minimum 2 years post graduate experience working with individuals with disabilities
  • Experience in mental health or dual diagnosis supports an asset
  • Experience in behavior supports an asset
  • Must have experience and skills in teaching and counseling individuals
  • Experience supporting individual with dual diagnosis along with behavioral management techniques is an asset
  • Various Programs require; experience with lifts, transfers, supporting individuals with high medical and high behavioral needs, understanding of protocols, crisis intervention, non-verbal/verbal, total care and semi-independent/independent living.
  • Attainment of a pharmacology certificate, Safe Management, First Aid and other related training required (if not, Vita will provide)
  • Valid G or G2 Driver’s License required
