This is a leadership opportunity for leading a multidisciplinary team focused in Seniors Care Advance Practice in the community. Under the direction of the Area Manager, the IHC - Seniors North Care Manager provides clinical and operational leadership to the Seniors North Team. The team is guided the McMaster Case Management practice principles and guides the client decision making. The position is responsible and accountable for daily management of staff and activities being provided to the clients living at home. The Care Manager will ensure that multidisciplinary staff of Registered Nurses, Occupational Therapists, Physiotherapist, License Practical Nurse, and Therapy Assistants maintain a high standard of care; that services are delivered using a client focused delivery method; that staff activities support program strategic directions; and that client needs are met in a fiscally responsible manner. The expectation is for Care Manager to collaborate with other Care Managers within the integrated Home Care team, with the Quality Education and Practice Leads for the Home Care Program, the Home Care Business Support Team and with contracted service providers who provide HCA and LPN services. The Care Manager will ensure program wide objectives are met through participating in a variety of committees, initiatives, program development opportunities and projects. The functions of the position are: leadership and coaching; collaboration; planning; service delivery; resource management; and coordination and communication. The successful applicant will be a part of the Management/Leadership group for the Seniors North and Dementia Care teams.

Experience with Case Management practice in the community, Baccalaureate degree in a health related discipline required. (Graduate level of education preferred.) Current registration with recognized professional body. Current BCLS-HCP registration required. Knowledge and experience in working with individuals, families and communities within Senior’s populations. Minimum of 5 years clinical experience required. Proven leadership through effective communication, ethical decision making and organizational skills. Monitors and ensures compliance with budgetary guidelines. Excellent communication and facilitation skills within a multidisciplinary team environment. Demonstrated conflict resolution and change agent skills. Basic computer skills are required. Proven critical thinking and decision making skills. Previous experience in assessing staffing needs, recruiting and hiring appropriate staff.
Additional Required Qualifications:
Minimum of 2 years’ experience in a management or leadership role preferred. Community experiences using the Case Management approach an asset.
