| Resident and Patient Relations Advisor St. Joseph ?s Continuing Care Centre ( www.sjccc.ca ) requires a Resident and Patient Relations Advisor who will be responsible for coordinating admission and discharges, coordinating care conferences, providing support and counseling for both families, residents and patients, acting as a liaison between residents, patients and families and the community, providing assessment, formulating goals and recommendations, planning and implementing client centered, goal-directed treatment programs for residents and patients requiring these services admitted to the Centre. The Resident and Patient Relations Advisor is involved in the education of residents, patients, family members, healthcare workers and the general public with regards to advocacy and the management of the resident ?s health related problems and needs. The Resident and Patient Relations Advisor will be expected to work closely with Programme Support Coordinator, Director of Care and the Director of Nursing in order to provide the highest quality of service.
