| Position Title: Registered Nurse - Care Planning Consultant Competition Number: CRH-12-116 Status: Part Time Regular FTE: .79 Department: Transition Team Position Type: Both Position Description/ Required Qualifications: ? Plays a key role in the coordination of quality integrated care planning and utilization management in collaboration with managers and care provider throughout Alberta Health Services. ? Demonstrates leadership in Wellness Independence philosophies and continuity of patient care by facilitating clients? movement across the continuum of care in a timely and seamless manner. ? Aware of ? Right Place , Right Time, Best Care? philosophy related to patient care. ? Liaises with community care staff and acute care staff through Emergency Room Response and daily inpatient unit contact in the Chinook Regional Hospital . Works well with a multidisciplinary team. ? Ability to prioritize workload when multiple demands occur and able to function in a challenging yet rewarding team environment. Participates in program development and staff education to promote quality discharge planning. ? Current RN Diploma, current CARNA registration, Nursing Degree preferred. ? Gerontology .... |
