| Title: Registered Clinical Dietitian Job Type: One Year Contract Location: Blenheim Community Village, Blenheim, ON Apply By: March 15, 2012 Responsibilities Completes initial assessment, including nutritional risk screenings on all new residents. Completes Resident Nutritional Assessments on residents identified at high nutritional risk. Makes recommendations regarding specific resident nutritional interventions and communicates with the Care Team regarding implementation of recommendations. Continued monitoring and follow-up as required. Assess and evaluate, on a regular basis, resident's need for changing nutritional needs (i.e. pureed diets, thickened fluids and nutritional supplements). Confers with the Resident's attending physician and other members of the multidisciplinary team, as necessary, regarding the nutritional plan of care and associated assessment needs, i.e.: blood work, diet changes. Participation as a member of the Interdisciplinary Team in the development and evaluation of specified nutrition care plans which includes an assessment of the resident?s needs, realistic goals and approaches, and an evaluation of the outcome of the nutrition care provided.
