MAJOR RESPONSIBILITIES: Assess each patient’s level of function and identify problem areas; Establish treatment goals based on assessment findings and then plan and initiate a treatment program that is continually monitored; Document initial assessment findings and at regular intervals reassessments and progress notes in patient’s medical records according to the Charting Policies and Procedures of the discipline and program; Be an active and consulting member of the Interdisciplinary team; Be involved in interdisciplinary committees and act as a consultant and supportive team member of other team members; Act as a consultant to allied health professionals family members; Delegate tasks appropriate to Physiotherapy Assistants knowledge base and monitors these activities; Attend in-services and conti.... |
