| PATIENT CARE COORDINATOR COMPETITION NO: 11-653 STATUS : Permanent Full Time DEPARTMENT : Medicine Inpatient Unit C-3N PROGRAM : Medicine Program REPORTS TO : Manager, 3N Med/Tele, D3M medicine, CCU, and the Cardiac Rehab HOURS : Days, 7.5 Hour Shifts JOB SUMMARY: The Patient Care Coordinator is a member of the program/clinical area management team and is responsible for the overall day-to-day unit-based coordination of patient and family care, facilitation of patient flow in and out of the unit, coordination of nursing staff and resources, and conduct of supervisory support functions such as attendance and performance management, and conflict resolution at the point of care and beyond. QUALIFICATIONS: Regulated Health Professional with Baccalaureate degree or currently enrolled in such a program. Current nationally recognized specialty certification program (preferred). BCLS current Demonstrated leadership skills with a minimum of two years of supervisory experience. Minimum of five years of relevant clinical care experience. Demonstrates strong clinical skills related to the area. Able to discern relevant information to make effective decisions regarding patient care planning. Kn.... |
