| Title: Nurse Practitioner Job Type: Full-time Location: Sumac Lodge, Sarnia, ON Apply By: March 1, 2012 Responsibilities With the Medical Advisory Physician (MAP), serving as a designated primary care provider to complete the following: performing rounds on each Resident Home Area with MAP once a month; ensuring MAP has reviewed and signed all Admission and Annual Physicals Exams; ensuring MAP has reviewed and signed all Quarterly Medication reviews and medications on admission contacting MAP regarding all acute care issues; Visiting individual Residents as frequently as deemed necessary by the Resident's health status and, in accordance with the home's policy, attending to any problems; writing appropriate orders and/or progress notes at the time of each visit. Liaising with local hospitals and outreach psychiatric services to set up protocols and channels of communication facilitating in-house problem management, transfer to hospital emergency departments and/or referrals to other community medical services. Participating as a member of the interdisciplinary team collaborating with Residents, families, physician and other members of the interdisciplinary team regarding the Reside.... |
