Halton Healthcare Services Posting #: 2012-670137 Position: Independent Dialysis Navigator Department: Renal Status: Part-time (availabile to work up to .5 FTE) Hours: Monday - Friday (flexible) (Hours are subject to change based on operational requirements) Site: Oakville Responsibilities: The Independent Dialysis Navigator is a member of the interdisciplinary team which supports patients in making informed decisions regarding their care. This person will take the lead in a comprehensive approach to all patients starting dialysis by liaising with the patient/family, Nephrologist and interdisciplinary team members with the purpose of providing education and support while encouraging independent dialysis modalities. Liaise with all patients/family and assess suitability for independent dialysis Provide education and decision support to patients/family who have not had the benefit of pre-dialysis modality education Coordinate/facilitate the smooth transition of patients into independent modalities by serving as an essential link between patients/family and the interdisciplinary team Identify and address barriers to independent.... |
