| Position Title Home At Last (HAL) Attendant Employee Group Casual/Relief Hours Per Week Vary On Call Yes Program or Dept. Community Support Services Salary Range 13.20 Job Summary The HAL Attendant provides personal support and homemaking services to individuals and families in the home within the framework of VON policies, procedures, standards and quality & risk management. Collaborates with members of the health care team in meeting the needs of individuals to attain an optimal level of independence and well being. Meeting client at hospital and supporting them through the discharge procedure and escorting them home. Assist clients to settle in following discharge from hospital. Light homemaking, meal prep, personal care if required, and completion of environmental risk assessments. Responsibilities ? Monitoring & Evaluation- Observes and evaluates the individual's/family's home, personal situation and client/family care plan ? Planning- Contributes to the development (with the help of the Home at Last Coordinator) of the personal support care plan ? Implementation- Performs personal support tasks reflecting the needs of the individual/family ? Communication Written/Verbal- Comm.... |
