| Guideline Implementation Facilitator Nursing Term Part-time (.4 EFT) 8 months Competition #: 122-10-765034960 Under the direction and leadership of the Knowledge to Action Initiative Lead(s), and reporting to the Clinical Practice Guidelines Initiative Coordinator, the Guideline Implementation Facilitator will drive the implementation of a supportive care practice guideline at CCMB and appropriate WRHA Community Oncology Program sites. The Facilitator is responsible for planning, coordinating, executing, and evaluating activities involved in implementing the guideline and for assisting an action research team studying the implementation process. Qualifications: Baccalaureate degree in health sciences or humanities preferred. Previous work experience in health care setting (minimum 2 years) required, clinical experience preferred. Awareness of, or experience in planning, implementing and evaluation of this type of health care initiative required. Experience in gathering, interpreting and collating health care literature and skill in pertinent computer applications required.
