| DIABETES NURSE EDUCATOR (Registered Nurse) Posted on Tuesday, January 17, 2012 Posted until: Saturday, March 17, 2012 Location: Stratford, ON POSTING #:8431 (ONA) HURON PERTH HEALTHCARE ALLIANCE NOTICE OF VACANCY POSITION: DIABETES NURSE EDUCATOR (Registered Nurse) Huron Perth Diabetes Outreach Team Full-Time Position POSITION SUMMARY: As a member of the Diabetes Education Team, the Diabetes Nurse Educator (DNE) will collaborate with the Diabetes Dietitian and our community partners to plan, develop, implement and evaluate diabetes education and self-management support for persons with diabetes in targeted communities within Huron and Perth Counties. This role encompasses weekly ?diabetes days ? in Clinton, Seaforth and Stratford diabetes education centres and community locations as required. The DNE will increase the awareness of diabetes and associated risk factors within the community at large and is a professional resource for health care providers in the area.
