| Clinician, Community Mental Health - Toronto, Ontario Back to Search Results JOB POSTING #671900 Position: CLINICIAN, COMMUNITY MENTAL HEALTH Site: TORONTO WESTERN HOSPITAL Department: GERIATRIC PSYCHIATRY - GERIATRIC MENTAL HEALTH OUTREACH TEAM Reports to: MANAGER Hours: 37.5 HOURS PER WEEK Status: TEMPORARY FULL-TIME 6 MONTH PILOT JOB SUMMARY: As an integral member of the Geriatric Mental Health Outreach Team, the principal responsibilities of the CLINICIAN, include providing comprehensive bio-psycho-social assessments of client off-site as residents in a LTC Facility, specifically focusing on those clients with difficult behaviors; providing treatment/ therapy for clients, providing therapeutic interventions for clients, families and significant others, on a short term basis utilizing a multimodal perspective; providing education and support in accordance with adult learning principles, and be prepared to mentor and coach direct care staff in LTC homes; collaborating and liaising with hospital/community groups including CCAC; attendance at ALC rounds from UHN and CAMH and to participate in quality improvement initiatives within the LTC homes that are serviced.
