| Position Title Care and Service Coordinator Employee Group Regular FT Hours Per Week 37.5 On Call Yes Category RPN/LPN Program or Dept. Peel Home Support Job Summary The successful candidate will coordinate and facilitate initial client visits, training of delegated tasks and the supervision of Personal Support Workers. Responsibilities The Care and Service Coordinator is responsible for all duties inherent to the position with enhanced responsibilities. They will be responsible for doing the following: Attending CCAC meetings and case conferences. Facilitate training of delegated tasks. Participate in initial client visits and give personal care to clients. Provide support as needed and ensure client safety and well-being is respected. Work with other members of the team to plan and/or facilitate the home support program. Work closely with Program Manager to meet criteria, guidelines and plan ongoing strategic direction of program. Work closely with Manager and team to review recruitment needs and take action based on requirements. Qualifications Certified as a Registered Practical Nurse. Some experience working with Personal Support Workers. Experience in case management and coor.... |
